You will check in at the front desk and complete any necessary forms or updates on the patient kiosk. After this you will have your vital signs (blood pressure, pulse, etc.) done. If any of your answers on forms, or the kiosk, need clarification this will be addressed by office staff if time permits.

In order to best utilize your time with Amber, there are forms you will be asked to complete before our first appointment. If you do not have access to the internet, or need help accessing the patient portal (access will be granted once we've agreed to schedule an initial evaluation appointment), please plan to arrive up to 30 minutes prior to your scheduled appointment so the staff may assist you with completing intake forms. A separate appointment with staff can be arranged.
Definition of a No Show: Failure to attend a scheduled appointment with Provider without 24 hours notice of cancellation, except in the case of an emergency or illness. It is the patient's responsibility to ensure they do not double book themselves and are able to attend their scheduled appointments with our Provider.
This fee will be due at the patient's next scheduled appointment. If it is not paid at that time, or before, any future appointments will be put on hold until this fee is paid or at least one installment on a payment plan has been made.

(Note: Medicare & Medicaid do not allow Providers to charge No Show fees. Two No Shows will result in a probationary period of up to 6 months of same day only appointments. If there is a transportation issue that would result in a No Show, you may avoid this by contacting your Provider's nurse before your scheduled appointment to switch to a phone or video appointment.)
**Repeated failure to attend appointments may result in termination of services at the discretion of the Provider, even if the patient gave sufficient notice to cancel or reschedule. Patients that do not engage in services for more than 6 months will not be guaranteed services or refills.**

You will be responsible for any costs not covered by your insurance, so check with your insurance plan about the % for which you are responsible. You will receive a bill from our office with balance owed, which is expected to be paid upon receipt. Payment plans are available for patients who need to budget their payments. *Co-pays are due at time of service*
*We will once again be expecting co-pays at time of service. See payment methods below. If you are unable to pay at time of visit you need to inform us in advance and create a reasonable payment plan.
It is your responsibility to request a copy of the Self Pay Fee Schedule prior to scheduling your first appointment. You will be responsible for paying the Self Pay Fee if you do not want your insurance billed by this office, if you have an insurance that we do not accept, or if you do not have health insurance.
These rates also apply if your health insurance is not active at the time of your visit. If you need assistance with insurance issues please notify our office at least 48 business hours in advance of your scheduled appointment (this does not guarantee a resolution to your insurance issue but will at least allow you to reschedule in advance, thus avoiding a No Show, if it cannot be resolved before your scheduled appointment).
Fees are subject to change at the discretion of the practice. Any fee changes will be posted at the office. If you have any questions about fees do not hesitate to ask the office staff.
Payment plans may be developed on an individualized basis. Please request a payment plan if you are unable to pay your bill in full at the time of service or when you receive your bill.
*Failure to adhere to an agreed upon payment plan may result in suspension of services until balance owed has been paid and/or termination of services at the discretion of the practice. If you find yourself struggling to keep up with payments do not postpone revising your payment plan with our staff.*
